Damian Mercer is living on the street dressed as a beggar. Under the name Umbra he ranks first among the physicians of the Sanctum, he is being hunted across the world, and the rags keep him out of sight. Liana Lowell, heiress to the Lowell family, takes him for a beggar and hands him a black card as charity. One look at her tells him she has Yin-collapsed meridians and that the condition is terminal. Three well-known doctors hear the diagnosis and dismiss it. He shows them up in front of everyone. After that he is taken to the Lowell estate whether he likes it or not, and with gold needles he saves Liana's life. The medical world hears about it and is shaken. He gets away from the estate, and straight away, in the street, his fiancée Molly Sawyer and his half-brother Rowan Mercer humiliate him in front of passers-by and break off the engagement. He finds out that the Mercer family now holds a seat among the Twelve Ascendant Elders of the Sanctum. The seat came through the Left Guardian of the Sanctum, who is Damian's own subordinate, and the family has grown arrogant on the strength of it. Damian begins laying plans. Under the name Umbra he announces that he will attend the Mercer family wedding, and the news causes an uproar in the city.
