Edwin lays a trap for his nephew in Calvin Village, and the nephew walks into it. What Edwin does not know is that the local troublemaker he grew up around is now Thomas, a traditional medicine graduate who woke up that morning inside him. Thomas treats Caroline after she is drugged, then treats enough of the village to work off the debt hanging over him, and goes in with Caroline and her mother on a shop selling medicinal tea. Edwin ends up in the hands of the law. Thomas moves to the city and opens a restaurant built around food cooked as medicine, feeds people who have come in from the countryside with nothing, and handles the rivals who want him gone. He gets the local magistrate's corruption into the open. When sickness spreads through a garrison camp he goes in and treats it, and the men there come round to him. An army crosses the border, Thomas is conscripted along with everyone else, and while he is with the army he turns up a plot by officials at court and gets the emperor out of it.
